This is NOT preventative care.🦷🚨‼️

I saw a 9-year-old patient as a temp.
Last year and put in my story …..

He had a “cleaning.” six months prior by the dental assistant and had an exam.

But here’s what didn’t happen 👇
❌ No x-rays for over a year and a half
❌ “Prophy” done without proper assessment
❌ Subgingival calculus… completely missed

When I scaled, I found black calculus below the gumline.
Yes ….. in a CHILD.

Let’s talk about what’s happening in dentistry right now.

Some states are pushing for OPA (Oral Preventive Assistants)
expanded-function assistants trained to perform basic preventive procedures like supragingival scaling

But here’s the reality:

👉 That role is designed for “healthy patients” and mild gingivitis cases only
👉 Training can be as little as ~120 hours

So let me ask you this:

What happens when disease is already BELOW the gumline… and no one is trained to look for it?

Because that’s exactly what I found.

👉 Children get subgingival calculus too.
👉 Disease does not stay above the gumline.
👉 “Above the gumline only” is NOT prevention.

That is how disease gets missed.
That is how this happens.

This is why the role of the Dental Hygienist is NOT replaceable

We are trained to:
✔️ Assess
✔️ Diagnose within our scope
✔️ Treat subgingival disease
✔️ Understand progression

Not just polish what you can see.

And THIS is why hygiene autonomy matters.

Because when hygienists have:
✨ Time
✨ Clinical control
✨ The ability to practice fully

We catch what others miss.

This isn’t about attacking new roles.
It’s about protecting standards of care.

Because if we lower the bar to “above the gumline only”…

👉 We are no longer preventing disease.
👉 We are allowing it to progress.

A 9-year-old should never leave an appointment labeled “healthy”…
when disease is sitting just beneath the surface.

👉 Dental Hygienists must advocate for autonomy so we can fully assess, diagnose within our scope, and treat what is clearly being missed.

Braces aren’t just about straight teeth… they’re about HEALTHY teeth too 🦷✨

I had a patient come in recently who just got braces , he was so excited to finally have them on, BUT he’s also a perio maintenance patient and hadn’t been able to get in for his cleaning, 🧼!

I was able to get him in ASAP, got everything cleaned up, and had a real talk with him…
now more than ever in braces his home care + maintenance visits matter.

Because brackets and wires trap plaque like crazy and if you already have a history of perio?

👉 inflammation + bone loss can progress FAST while teeth are moving.

• Electric toothbrush with timer (full 2 minutes)
